How to Fish Island Progression

How To Fish 3 Leeches

Quick answer: the leech quest needs exactly 3 leeches — not 4, not 5. They are small ground pickups on Island 2 (Forest), not fish you hook and not boss drops. Collect all three, hand them to the woman wearing a hat by the lake, and she gives you the Modified Leech — the bait that summons the Giant Piranha, the game's second boss.

Where the leeches are

The leeches sit on the ground around Island 2's usable paths, near the lake and the quest giver's area. Because the models are tiny and the island is visually busy — trees, props, vegetation — the search is a scanning problem more than a walking problem. Lower the camera toward ground level and sweep outward from the quest NPC instead of running the same center path repeatedly; the pickups most commonly missed are the ones near the perimeter edges rather than the ones in open ground.

A clean collection route: talk to the hat woman first so the 0/3 counter is active, then walk one full loop of the island's walkable edge, checking beside structures and vegetation as you go. Check the counter after every pickup and stop at 3/3 — continuing to search for a fourth is the most common time-waster on this quest.

Common leech questions

A few questions come up on every leech thread. Can you fight the Giant Piranha without all three? No documented route exists — the Modified Leech is only issued at 3/3, and there is no alternative boss bait for the lake. Do the leeches respawn if you drop one? The safer assumption is yes-but-reseed: dropping a quest pickup puts it back into the world pool, and a fresh session restores missing counts, but nobody has documented a same-session respawn timer, so plan around one re-entry at most. Can a teammate collect them for you in co-op? The counter is shared across the lobby, and any player's pickup advances it — split the island into zones and the search takes a third of the time. And does the quest expire? No timer is documented; the hat woman waits as long as your save does.

Who takes the leeches, and what you get

The turn-in is the woman with the hat standing by the lake — Game8's guide names her the Daughter NPC, and the community just calls her the hat lady. Hand over all three and she returns the Modified Leech, a bait item rather than a normal lure. Equip it on your fishing rod and cast into the lake: that cast summons the Giant Piranha, the second boss. This is the same bait-for-boss pattern as the first island's beer can, and it repeats across the whole game — quest item in, boss-summoning bait out.

Two cautions from players who lost progress here: do not sell or eat the leeches before the hand-in (they are quest items, not food), and confirm the Modified Leech is in your bait inventory before you leave the NPC — if the counter hit 3/3 but you got no bait, talk to her again before sailing anywhere.

If the counter stops below 3/3

Players have reported incomplete leech spawns — the counter stuck at 1/3 or 2/3 with no pickups left to find. Before assuming a bug, do one slow perimeter sweep at ground level; the third leech hiding behind a prop is far more common than a genuine spawn failure. If the sweep genuinely finds nothing, the community's working fixes are unremarkable but real: re-enter the island (sail away and return) or restart the session, which reseeds pickups. What does not work, reliably, is waiting — the spawn does not appear to refresh on a timer.

If you lose a leech after collecting it — dropping it, dying with it, selling it by accident — the same re-entry logic applies: the quest state remembers the count, and a fresh session restores any missing pickups. Protect the save before experimenting: the save file lives in AppData and a manual backup costs ten seconds.

What the leeches unlock

The chain is: 3 leeches → Modified Leech → Giant Piranha fight → Piranha Skeleton → hand the skeleton in to receive Island 3's coordinates. In other words, these three pickups gate the entire mid-game — you cannot reach the Desert island, the Grill, or the Pufferfish without them. That makes the leech quest the first place the game teaches its core rule: quest drops are progression currency, and selling a quest item can soft-lock an afternoon.

Fight prep while you are on the island: the solo community strategy for the Piranha is a shotgun on the boss plus melee for the small piranhas it spits, and the shortcut is store-bought dynamite at 25 a piece. Both routes are covered in the full boss guide.

What not to do

  • Do not search for more than 3 — the objective caps at 3/3 by design.
  • Do not hook for leeches in the water — they are ground pickups, and casting wastes time.
  • Do not sell, cook or drop the leeches before the hand-in.
  • Do not sail to another island mid-quest to "refresh" spawns without noting your count — re-entry resets pickups, not your progress.

### Do leeches count toward anything else?

No — the leeches are pure quest items. They do not sell for meaningful money, they do not cook into anything worth eating, and no achievement tracks them. Their entire value is the Modified Leech at the end of the chain, which is why the standard advice is to stop searching the moment the counter reads 3/3 and walk straight to the hat woman.
