Step 1: Beat the Giant Piranha

Common Ways Players Get Stuck
The number one blocker is selling the skeleton as meat, which leaves you with a dead boss and no coordinates. The fix is unglamorous: fight the Giant Piranha again and loot properly. The number two blocker is fishing for leeches in the water, because they are ground pickups, not catches.
A smaller group of players beat everything, grab the skeleton, and then wander off without the hand-in. The coordinates only exist as a quest reward, so the NPC delivery is mandatory. Slow down after the kill and the island opens itself.
A final reassurance: nothing about this unlock is permanently missable. The boss can be re-summoned with another leech bait, the skeleton will drop again, and the NPC is not going anywhere. The only real cost of a mistake is time — one more leech run and one more fight.
